Please take a moment to comply with this very important request.
We highly recommend that you SUBMIT YOUR CHILD's ANNUAL EXAM AND IMMUNIZATION RECORD each and every year. This allows your child to remain in school, participate in physical education, fulfill necessary mandated screenings, and meet immunization requirements.
In addition, 7th and 8th-grade students who want to participate in our modified sports program will require a recent physical on file. The exam must be less than one year old and be performed by a NY/NJ/CT physician.
All reports can be mailed, emailed to lfaneca@scarsdaleschools.org, or faxed to 722-2850. (We are open all summer and can receive faxes 24/7.)
