• New York State Requirements
     
    We highly recommend that you SUBMIT YOUR CHILD's ANNUAL EXAM AND IMMUNIZATION RECORD each and every year. This allows your child to remain in school, participate in physical education, fulfill necessary mandated screenings, and meet immunization requirements. 
     
      
    ALL students must meet the NYS Immunization Requirements
    .

    4 -5 doses of Diphtheria Toxoid (usually administered as DPT, Dt, DtaP or TD).
    3-4 doses of Poliovirus vaccine (OPV, IPV or eIPV).
    3 doses of Hepatitis B (In accordance with ACIP schedule)
    2 doses of Measles vaccine, the first administered after 12 months of age (most often given as MMR)
    2 doses of Mumps vaccine administered after 12 months of age (most often given as MMR)
    1 dose of Rubella vaccine administered after 12 months of age (most often given as MMR)
    2 doses of Varicella (chicken pox) vaccine (or MD documentation of disease)
    1 dose Tdap given at 11 years of age
     
    Tuberculin Testing is recommended. Serologic evidence is now acceptable for MMR, Hep B and Varicella. 
    Foreign documents must be translated into English.
     

    Other requirements by grade:

    Physical Exam for all new entrants and students in grades K, 1, 3 and 5:

    A physical exam is required for all new entrants and students in Kindergarten, 1st, 3rd and 5th grade. This exam must be performed by a physician practicing within 50 miles of the NY state borders. It must have been performed within the past 12 months.
     
    ALL kindergarten students must have a physical and immunization record on file prior to entry on the first day of school.
     
    All new physicals must be submitted on the mandated NY STATE physical exam form. This form is available in our offices and through this link: Health Examination Form.  Pediatric practices also have these.  Physicals submitted on other forms or attached to the new form but left blank cannot be accepted and will be returned.

    Please download the Health Examination Form 
    for your physician to complete. This form should be submitted within 14 days of the start of school in September. Those students from out of state and/or out of country have 30 days to submit. Your child MAY NOT participate in physical education without this on file.
     
     
    Vision and Hearing screenings are required during the school year in Kindergarten and grades 1, 3, and 5.  The nurse performs these exams and notifies parents of any abnormal findings.


    Scoliosis screening grade 5:
    A scoliosis screening must be performed on all girls in grade 5.  A copy of the child's most recent physical exam with the scoliosis results noted is acceptable (Health Examination Form).  Any student that does not have a recent physical on file will be screened by the nurse.  Parents will be notified of any abnormal findings.
