Sports Clearance
In order for seventh and eighth graders to participate in the modified sports program, we require a physical exam (less than one-year-old) and a completed health history and parental consent form(s). In addition, if your child is prescribed Epinephrine (Epipen, Auvi- Q, etc) for allergies, they must also have an Allergy Action Plan on file with the health office. If your child is prescribed an inhaler, they must have an Asthmatic Reaction Procedure on file with the health office. Each of those documents must be submitted each school year. The consent form must be completed by a parent in ink. The sport must be noted. Once the health office has all the required paperwork, the office reviews all documentation. If the student can safely participate in the chosen sport and paperwork is in order, they are cleared and added to the roster.Please refer to the Modified Sports web page for specific instructions and forms. The program is NOT open to sixth graders.
